Glossary of Terms

The NHS and the world of PFI are both very good at using jargon and acronyms. Please use the following glossary to find their meaning. If you cannot find the meaning of a word you have come across on this website then let us know by clicking on ‘Your Questions’ and sending us a question.

A&E

Accident & Emergency

ACC

Ambulatory Care Centre.

Acute Care

Care provided for a patient for whom a treatment plan is in place. The patient should be medically stable but requires regular medical / nursing observation, assessment and intervention.

ADSCR

Annual Debt Service Cover Ratio.

Advisers

The organisations appointed by the Trust to provide professional advice on the procurement process for the Scheme.

AEDET

Achieving Excellence Design Evaluation Toolkit; a toolkit for evaluating the design of healthcare buildings.

AHP

Allied Health Professional.

Assessment

A process, which may occur at any point in an episode of care, by which health professionals, either singly or working as a team, draw conclusions about a patient’s needs and devise a plan for clinical investigations, treatment and care.

Assessment and Critical Care

Care provided for a patient who is (potentially) medically unstable, requiring continuous review, nursing and medical intervention.

Availability Deduction

Deductions from the Unitary Payment made by the Trust in relation to the non availability of Functional Areas / Units in accordance with the Payment Mechanism.

Backlog Maintenance

Maintenance required on the Trust’s estates that has not been undertaken in accordance with the planned lifecycle and which has therefore built up into a ‘backlog’ of maintenance requirement.

Bedding-In Period

Agreed initial period during which the full impact of deductions under the Payment Mechanism is not implemented.

Bid

A response to the ITN containing an offer on the part of a Bidder to undertake the Project Operations.

Bidder

Reference to any one of the Bidders.

Bidders

Together, the Bidders to whom this ITN has been issued and who are therefore invited to submit a Bid.

Bill of Quantities

List of the equipment required (type and quantity) within the New Facilities.

BMA

British Medical Association.

Bulk Transfers

Special arrangement whereby enhanced transfer terms are negotiated with the Project Co to allow the transferring members to transfer accrued pension benefits to the Project Co pension scheme and receive pension benefits of equivalent value to those earned in the NHSPS immediately before transfer.

C/E

Consulting / Examination.

CABE

The Commission for Architecture and the Built Environment.

CAM

See Capital Accounting Manual.

Capital Accounting Manual

Procedures for accounting for fixed assets in the NHS, as published by NHS Executive.

Capital Charge

The depreciation and capital cost absorption duty associated with the Trust’s ownership of assets in accordance with the Capital Accounting Manual.

Care

One or more interventions, by one or more Trust staff, that is intended to provide physical or emotional support, assistance and comfort. It includes measures to encourage self care, promote health and to prevent loss of function.

Care Trust

An NHS Trust designated as a Care Trust pursuant to section 45 (1) of the Health and Social Care Act 2001.

Car-parking Contract

Existing contract that operates in relation to Trust car parks.

Car Parking Management Service

The services undertaken by the Trust in conjunction with the Car-parking Contractor in relation to the day-to-day operation of the Trust’s car parking.

Car-parking Provider

Provider of Car-parking facilities being Gentian (Mid Essex) Limited.

CBC

Chelmsford Borough Council being the Local Planning Authority for this Scheme.

CCU

Cardiac Care Unit.

CHAD

Centre for Health Architecture and Design.

Change Control Process

The formal process by which the Trust will record, evaluate, authorise and communicate changes to the scope of the Scheme.

CHC

Community Health Council.

Checkpoint 1

Point in the development of the Bid Submission at which Bidders make an interim Submission to the Trust. For Checkpoint 1, this Submission  concentrates upon design development.

Checkpoint 2

Point in the development of the Bid Submission at which Bidders make an interim Submission to the Trust. For Checkpoint 2, this Submission  concentrates upon legal and commercial matters.

Checkpoint 3

Point in the development of the Bid Submission at which Bidders make an interim Submission to the Trust. For Checkpoint 3, this Submission  concentrates upon design development with an update on legal and commercial matters.

CIM

The Capital Investment Manual as published by NHS Executive.

CLCR

Concession Life Cover Ratio.

Clinical Equipment

Equipment utilised by the Trust for undertaking clinical services; equipment directly used in the treatment of patients.

Clinical Networks

Networks of healthcare specialism established across traditional organisational boundaries (eg cancer networks).

Clinical Working Groups

User focus groups established by the Trust to aid the process of Bid clarification and evaluation.

COG

Clinical Oncology Guidelines.

Commissioners

The authorities (Strategic Health Authorities or PCOs) which commission clinical services from the Trust.

Concession Period

The operational period of the Project Agreement; the period during which the Project Co. provides services to the Trust.

Commercial Close

The date on which the Trust and the Project Company enter into the Project Agreement.

Composite Trader Tax Treatment

Tax treatment which can be applied to PFI schemes and which may enable Project Co to significantly reduce their tax burdens.

Condition Survey

Survey to be undertaken on a jointly agreed scope between the Trust and the Bidders with the objective of assessing the current condition of the Retained Estate and identifying the Backlog Maintenance Works needed to bring the Retained Estate up to Estate Condition B.

Contract Debtor Accounting

Accounting treatment of Project Co whereby the PFI facility is accounted for on the balance sheet as a financial asset which is amortised over the life of the Project.

Contract Month

A calendar month.

Consumerism

Ways to help Trusts improve patient satisfaction during their interaction with the NHS.

Core Services

Those Soft FM services subject to Retention of Employment.

CT

Computed Tomography (commonly known as CAT scan).

CWGs

See Clinical Working Groups.

Data Room

Source of unwarranted additional data on the Trust and the Scheme.

DCAG

See Departmental Cost Allowance Guides.

Decanting Facilities

Temporary facilities into which the Trust’s services will move during relevant phases of the construction of the New Facilities.

Departmental Cost Allowance Guides

Standard construction cost guidance as issued by NHS Estates.

Department of Health

The Government department with responsibility for delivering NHS policy as agreed by the Secretary of State for Health.

Design Champion

Non Executive Director of the Trust charged with championing the cause of good design at all stages and levels of the Project. For this Scheme the Design Champion is Liz Fraser.

Design Quality Evaluation Toolkit

Design Review Panel

Process established to provide advice, support and guidance to NHS Trust’s and PCO’s in the form of a design review and subsequent report. Panel is chaired and supported by staff from the Centre for Health Architecture and Design (CHAD).

Development Plan

Plan agreed between the Trust and the Preferred Bidder that ensures the development of service standards as applicable to the Interim Services toward the Service Level Standards required by the Output Based Specifications for the Hard and Soft FM Services.

Development Site

The area of Broomfield Hospital site on which the New Facilities will be developed.

Diagnosis & Treatment Centre

New way of delivering elective care so that it is insulated from the pressures of emergency services.

DGH

District General Hospital.

DOH

Donated Assets

Assets that are provided to the Trust as gifts or purchased via charitable funds.

DQET

DRP

Initial works required to be undertaken on the Broomfield Hospital site in order to allow the construction and operation of Decanting Facilities, New Facilities and Retained Facilities.

Equipment Replacement Plan

Plan that describes the timescales in which the Trust’s equipment will be replaced.

ESSA

Essex Shared Services Agency.

Estatecode

Publication from NHS Estates which provides guidance to NHS organisations on managing their estate.

Estate Condition B

Assessment of the condition of the estate in accordance with Estatecode, being an operationally acceptable standard for all building and engineering elements.

Evaluation Panels

Panels established by the Trust to undertake the evaluation of Bid Submissions to the ITN.

Existing Facilities

The facilities utilised by the Trust for current operations.

External Stakeholders Group

Group constituted to represent the interests of local government organisations and other health organisations in the Project.

FBC

FCEs

Finished Consultant Episode’s.

Financial Advisers

The advisers appointed by the Trust to provide financial advice on the Scheme procurement process; for this Scheme the Financial Advisers are Ernst and Young.

Financial Close

The date at which the Project Agreement becomes unconditional. This should be assumed to be co-terminous with Commercial Close.

Financial Model

Computer spreadsheet model detailing the expenditure, revenues, financing and taxation of the Project Co and associated profit and loss accounts and balance sheets.

Firecode

Health Technical Memorandum (HTM 81-87) describing the fire strategy and planning issues to be met by NHS properties.

FM

Facilities Management.

FM Service Facilities

Those elements of the New Facilities and Retained Facilities that are utilised for the provision of FM services.

FRS5

Financial Reporting Standard 5 (Reporting the Substance of Transactions) as issued by the UK Accounting Standards Board.

Full Business Case

The Full Business Case which is required to support the Project and which will be published after receiving HM Treasury approval, after Financial Close.

Functional Area

An area of the New Facilities or Retained Facilities comprising two or more Functional Units.

Functional Unit

A room or space within a Functional Area.

Funders

All or any of the persons who provide financing or funding in respect of the Project Operations under the Funding Agreements.

Funding Competition

A competitive process undertaken by the Bidders in order to secure the most advantageous terms of funding for the Scheme. This process will be overseen by the Trust’s Financial Advisers.

Funders’ Direct Agreement

The agreement to be entered into between the Trust, the Senior Funders and the Project Company.

Funders’ Due Diligence

Review process undertaken by the Funders to achieve assurance that their level of investment is suitable in the circumstances of the Scheme.

Funding Agreement

All or any of the agreements or instruments to be entered into by the Project Company or any of its associates relating to the financing of the Project Operations.

Funding Competition

A competition run by the Bidders between funding solutions in order to identify the funding package for the Project which offers best value for money.

GAD

Government Actuaries Department.

GAD Passport

Advance certification provided by GAD to organisations who regularly bid for PFI projects, that their pension scheme satisfies the ‘broad comparability’ test for a specified category of employee.

GI

GastroIntestinal.

Governance Plan

The Trust’s annual plan for achieving Clinical Governance requirements.

GP’s

General Practitioners.

Group 1 Equipment

Items which are normally supplied and installed by the Project Company under the terms of a building/engineering contract with the Trust and funded within the works cost and which are generally fixtures, fitted furniture and items of plant or equipment which are permanently wired or installed.

Group 2.1 Equipment

Large items of fixed equipment which are usually provided by the Trust under the terms of a building/engineering contract with the Trust and which have a specific building construction and engineering service requirement for their installation and which are put in place by the Project Company.

Group 2.2 Equipment

Items of fixed equipment which are usually provided by the Trust under the terms of a building/engineering contract with the Trust, which do not have a specific building construction and engineering service requirement for their installation, and which are put in place by the Trust.

Group 3 Equipment

Moveable pieces of furniture and equipment required for the performance of clinical and non-clinical services which are usually purchased by the Trust under the terms of a building/engineering contract with the Trust and put in place by the Trust once the facility has been handed over.

Group 4 Equipment

Consumable items of equipment which are normally supplied and installed by the Trust.

GU

Genito-Urinary.

Hard FM Services

The maintenance of all buildings, fabric, engineering services, energy supply infrastructure and associated landscaping.

GUM

Genito-Urinary Medicine.

HDU

High Dependency Unit.

Hospital Non-Acute

The patient needs continuing assessment and therapeutic interventions within a treatment plan. The patient is medically stable, requiring periodic review by a hospital consultant, or at the request of the nurse or allied health professional.

Housing Provider

The Housing Association with whom the Trust will contract for the provision of staff residential accommodation.

HSDU

Hospital Sterilisation and Disinfection Unit.

ICAO

International Civil Aviation Organisation.

ICRS

Integrated Care Records Service.

ICU

Intensive Care Unit; see also ITU.

IM&T

Information Management and Technology.

Improving Working Lives

National standard for making improvements in the working lives of NHS staff.

Income Generation Schemes

The provision of Retail Units or other commercial facilities by the Preferred Bidder as part of the Scheme with the resulting financial benefits shared between the Trust and the Project Company on some agreed basis.

Inpatient

A patient who has gone through the full admission procedure and is occupying a hospital bed.

Input Specification

Service specification relating to the provision of Interim Services.

Interim Services

The provision of Hard FM and / or Soft FM services to the Trust post Financial Close but prior to commencement of operation of the New Facilities.

IOL

Induction of Labour.

IRR

Internal Rate of Return.

Issues

Those questions and requests for information generated within the Trust, which are recognised as being issues and which may become Requests for Change.

ITN

Invitation to Negotiate.

ITU

Intensive Treatment Unit. Alternatively known as Intensive Care Unit (ICU).

IWL

JCNC

Local Improvement Finance Trust; a Department of Health initiative to develop and encourage a new market for investment in primary care and community based facilities and services.

Loan Life Cover Ratio.

Local Planning Authority

For this scheme Chelmsford Borough Council (CBC).

LSCS

Lower Section Caesarean Section.

LSP

Local Service Provider, in the context of the National Programme for IT.

Managed Equipment Service

A service provided by the Project Company to the Trust whereby the provision, repair, maintenance and replacement of equipment is undertaken on behalf of the Trust by a Project Company for a standard unitary payment.

Mandatory Bid

A Bid which satisfies the requirements set out in section 1.12.2 of this ITN.

Master Plan

A requirement of the planning brief, establishing a requirement for a plan for the cohesive development of the whole Broomfield Hospital site.

MEHST

Mid Essex Hospital Services NHS Trust; the Trust.

Memorandum of Information

Outline information on the Scheme, as issued to those organisations expressing an interest in the Scheme following publication of the OJEC advertisement.

MEL

The Trust’s Medical Equipment Library; source of generic medical equipment which is centrally held and then loaned to individual wards and departments.

NHSE

The National Health Service Executive.

An executive agency of DOH established to provide information and guidance on estates and facilities management issues.

NHS Foundation Trusts

NHS organisations established as free-standing legal entities not directly subject to the Secretary of State’s powers of direction.

NHS Performance Ratings

The grant of outline planning permission by Chelmsford Borough Council.

Outpatient

A patient attending for treatment, consultation and advice but not staying in hospital.

Output Based Specification

The document which describes the Trust's output based requirements for a service.

PACS

Picture Archiving and Communication Systems.

PAS

Patient Administration System.

PBR

Payment by Results

Payment Mechanism

The mechanism by which the Trust will pay and incentivise the performance of the Project Company during the Project Term.

PCO

Primary Care Organisation; a generic term for Care Trusts and Primary Care Trusts.

PCR

Polymerase Chain Reaction.

PCT

Performance Monitoring System

System by which the performance standards of FM Services are monitored and measured, for interaction with the Payment Mechanism.

PEAT

Patient Environment Assessment Teams.

PET

Positron Emission Tomography.

PFI

PFU

The Private Finance Unit of the Department of Health.

PITN

Preliminary Invitation to Negotiate.

Planning Brief

Document provided by CBC as part of the Outline Planning Permission; forms the framework by which the Master Plan will develop.

PMS

See Performance Monitoring System.

Policy Board

Joint clinical and executive management board of the Trust that approves future policies and procedures.

PPE

Post Project Evaluation.

PPU

Private Patients Unit.

PQQ

The Pre-qualification Questionnaire issued to those organisations expressing an interest in the Scheme following publication of the OJEC advertisement.

Preferred Bidder

The Bidder with whom the Trust elects to proceed to Financial Close.

Primary Care Trust

A Primary Care Trust established under Section 16A (1) of the National Health Service Act 1977.

Private Finance Initiative

The UK Government’s Private Finance Initiative or similar initiative replacing the Private Finance Initiative.

Process Board

Forum established by the Trust to identify and agree future clinical processes.

Project

The Project as contemplated by this ITN. See also Scheme.

Project Agreement

The legal agreement reached between the Trust and the Bouygues Consortium being the Standard Form Project Agreement version 3, as adjusted for project specific requirements.

Project Board

Oversight Board of the Trust that provides overall direction and management of the Project.

Project Co

See Project Company.

Project Company

The private sector company with whom the Trust will enter into the Project Agreement.

Project Director

Officer of the Trust who ensures the Project remains on target to deliver the expected products; for this Scheme Malcolm McKillop.

Project Executive Lead

The Trust’s Executive lead for the Project; for this scheme the Trust’s Director of Finance; Phil Neal.

Project Operations

The carrying out of the works, pre and post completion by the Project Company, the maintenance of the facilities, the management of the services and the performance of all other obligations of the Project Company.

Project Sponsor

For this Project, the Trust’s Chief Executive; Andrew Pike.

Project Term

The period commencing on the date of the Project Agreement and expiring on the date which is equivalent to the construction period and a Concession Period of thirty years from the date of the Project Agreement or on such other date as may be determined in accordance with the Project Agreement.

Project Website

Proposed Workforce

The workforce proposed by the Preferred Bidder being that required to deliver the Hard and Soft FM Services for the Scheme.

Requests for Information

Requests for additional information or clarification posed by the Bidders.

Residential Accommodation

Separate Project for the provision of Residential Accommodation available to staff at below market rents.

Residential Property

Individual unit of property identified for Residential Accommodation.

Response

A Response from a Bidder to this ITN in the form described by the Information Request.

Response Submission Date

The date by which all Responses to the ITN must be received by the Trust (01/2012/2003).

Retail Units

Commercial facilities such as shops and restaurants provided by Bidders as part of any agreed Income Generation Schemes.

Retained Employees

Those employees who will be seconded from the Trust to the Project Company under Retention of Employment as part of the Proposed workforce.

Retained Facilities

The elements of the Existing Facilities which will continue in use by the Trust and not be subject to demolition or disposal.

Retained Services

Those non clinical services which the Trust will continue to provide itself and which therefore do not directly form part of the Scheme.

Retention of Employment

Process by which staff employed in the 5 Core (Soft FM) Services remain as NHS employees despite the transfer of the service they provide to the Project Company.

RoE

Room Data Sheets

Detailed design schedules agreed for each room within the New Facilities.

RFI

RPI

Retail Prices Index (All Items) as published by the Office for National Statistics.

RSL

Registered Social Landlord.

Service and Financial Framework.

Special Care Baby Unit

Schedules of Accommodation

Schedules of the Trust’s anticipated accommodation requirements for this Scheme.

Scheme

The Scheme as contemplated by this ITN. See also Project.

Section 106 Agreement

An agreement under Section 106 of the Town and Country Planning Act 1990.

Services

The services to be provided and/or procured by the Project Company for the Trust in accordance with Schedule 14 of the Project Agreement.

Service Level Standards

The individual service standards defined within the OBSs for Hard and Soft FM Services.

Service Providers

Persons engaged by the Project Company to procure the provision of the Services.

Shadow Unitary Tariff

An approximation of the annual unitary payment generated by the PFI scheme, based upon the capital and revenue costs of the Public Sector Comparator.

SFPA

Standard Form Project Agreement as published by the Department of Health’s Private Finance Unit.

SOC

Soft FM Services

Those FM services that are not Hard FM, being FM services supplied as integral to the operation of the Facilities.

Strategic Outline Case

The underlying strategic case that identifies the need for the Project to proceed. Requires approval prior to the OBC stage.

Technical Advisers

The advisers appointed by the Trust to provide technical advice on the Scheme procurement process; for this Scheme the Technical Advisers are Mott MacDonald.

Technical Groups

Groups established by the Trust to undertake the technical aspects of evaluating the Responses to the ITN.

The NHS Plan

The overall Department of Health policy document for the development of the NHS over a 10 year period; published in 2000.

The Prince’s Foundation

Charity that combines HRH the Prince of Wale’s interests in art, architecture and the built environment.

The Trust

Mid Essex Hospital Services NHS Trust; MEHST.

Transferred Employees

Those employees who will transfer from the Trust to the Project Company under TUPE as part of the Proposed Workforce.

Transfer Regulations

Treatment

One or more interventions, by one or more health professionals, that is intended to alleviate or modify the cause or symptoms of illness or disability.

Trust Board

The Board of Directors of the Trust; both Executive and Non Executive Directors. The Trust Board meets on a monthly basis.

Trust's Construction Requirements

The requirements of the Trust set out in the Project Agreement as amended from time to time in accordance with the terms of the Project Agreement.

Trust Members

Members of the Trust Board.

Trust Representative
